Requests for categories other than employee illness require a comment in the "Notes to Administrator" box.
Use of bereavement and family illness require notation of the familial relationship.
-Life Event (ex: divorce, marriage, birth, death) MUST be reported within 60 days of event. If it's over 60 days, changes can be made during Open Enrollment.
-Open Enrollment is April 22, 2024 - May 10, 2024. Changes are effective July 1
Hired on or after 07/01/2020, enrollment choices are Chap 44 - New Jersey Educators Health Plan (NJEHP) or Garden State Plan (GSP)
