The CompleteCare Health Center at Vineland High School Tanner Building (9-10 building) is now available for summer sports physicals. Any Vineland middle or high school student who needs entry physicals, sports physicals, or working paper exams is welcome to come by the health center between 8am-12pm. No appointment is needed; walk-ins are welcome. Please bring an insurance card (private or NJ Medicaid) and any updated vaccine records for Sports Physicals. If not yet registered for NJ Family HMO plan a CompleteCare staff member will assist with the process. All students younger than 14 must be accompanied by an adult. All students 14 years old and older must have a completed and signed CompleteCare application form to be seen without a parent present. For more information please call 856-794-6928.
